Please note registration starts at 7:30am. Books written by Dr. Burns will be available for purchase.

 

Workshop Registration Terms and Conditions:

Seat(s) confirmation is based upon receipt of payment & registration together by office.

Registration Fee Includes:

• Dr. Burns' Cognitive Behavioural Therapy Four-Day Intensive certificate of completion
• Reference notes

Prices include 5% GST.

Continuing Education Credits: Canadian Psychological Association, Canadian Counselling Association, Canadian Addiction Counsellors Certification Federation, Association of Social Work Boards, Canadian Association of Rehabilitation Professionals
Approved for 30 credit hours.

Cancellations: If you are unable to attend, you are invited to send an alternate in your place, at no extra cost. Please notify us of the name of the alternate, so that we may correct our records and minimize confusion at check in.
Refunds will be available, minus $75 administration fee, 30 days or more prior to the workshop. For cancellations less than 30 days prior to the event, a full credit will be available towards future Jack Hirose & Associates Inc. training. For cancellations less than one full business day
prior to the workshop, we regret that a refund or credit will not be offered.
Our liability is limited to refunds for workshop fees only. Jack Hirose & Associates Inc. reserves the right to cancel the workshop. Please make hotel & travel arrangements with this in mind.
Note: Jack Hirose & Associates is not responsible for any presenter’s or participant’s statements, acts, materials, or ommissions. The use of audio and video taping devices, beepers, and cell phones is not permitted at any workshop. Children and unregistered guests are not permitted in the meeting rooms.
